We are looking for a highly organized, compassionate and proactive Care & Growth Coordinator to support our clients, caregivers and continued growth.
This is a dynamic position combining client intake, care planning, scheduling, caregiver recruitment, client relationship management and community outreach/business development.
The Role
The Care & Growth Coordinator will be a key connection between Adokam Home Care, our clients and families, caregivers, prospective clients and community referral partners.
The successful candidate will help ensure our existing clients receive excellent service while supporting the continued growth of the company.
Your Role
1. Client Intake & Coordination
- Respond to new inquiries and assess client needs.
- Schedule consultations and coordinate new-client onboarding.
- Follow up with prospective clients and maintain intake records.
2. Care Planning
- Conduct/coordinate client assessments.
- Develop and update individualized client care plans.
- Communicate care needs and changes to caregivers and families.
3. Client Relationship Management
- Maintain regular contact with existing clients and families.
- Conduct client check-ins and monitor satisfaction.
- Address concerns and coordinate changes in care.
4. Caregiver Scheduling & Coordination
- Create and maintain caregiver schedules.
- Match caregivers appropriately with clients.
- Manage shift changes, call-outs and coverage.
5. Caregiver Recruitment & Hiring
- Recruit, screen and interview caregivers.
- Coordinate references, documentation and onboarding.
- Maintain an adequate pool of available caregivers.
6. Community Outreach & Growth
- Build referral relationships with hospitals, clinics, social workers and community organizations.
- Conduct outreach, meetings and follow-ups with potential referral partners.
- Track leads and referrals and help convert opportunities into new clients.
Responsibilities include:
- Conduct outreach to hospitals, medical clinics and healthcare organizations.
- Introduce Adokam Home Care to appropriate healthcare and community professionals.
- Build relationships with social workers, discharge-planning teams, case managers and other relevant professionals.
- Develop relationships with family physician clinics, rehabilitation providers, physiotherapists, occupational therapists and other healthcare providers.
- Connect with senior-serving organizations, retirement communities, disability organizations, churches and community organizations.
- Make introductory calls and send professional outreach correspondence.
- Arrange meetings and presentations with potential referral partners.
- Attend selected networking and community events.
- Distribute approved Adokam informational materials.
- Maintain an organized database of referral contacts.
- Conduct regular follow-up with referral partners.
- Track outreach activities and referrals received.
- Identify potential partnership and business-development opportunities.
- Represent Adokam professionally within the community.
Digital advertising and campaign management are handled by our marketing specialist. The Care & Growth Coordinator will focus on responding to and converting incoming opportunities while developing relationships that can generate additional referrals.

Qualifications
Previous experience in home care, healthcare, care coordination, client intake, care planning, scheduling, healthcare staffing, recruitment or community outreach is strongly preferred.
Experience developing or working with individualized client care plans is considered a strong asset.
A healthcare background, including HCA, LPN, nursing, social services or related experience, is considered an asset.
Experience with home-care scheduling software, CRM systems or electronic client-management systems is an asset.
Strong verbal and written English communication skills are required.
Knowledge of Airdrie, Calgary and surrounding communities is considered an asset.
A valid driver's licence and reliable access to transportation are preferred due to community outreach and potential client visits.
Hours & Compensation
$20.00 per hour
Approximately 20 hours per week.
Regular weekday daytime availability is required because the position involves communication with clients, caregivers, prospective families, healthcare professionals and community organizations.
The role includes a combination of remote administrative work, client/caregiver coordination and community-based activities.
Some flexibility may occasionally be required for urgent scheduling matters.
There is potential for increased hours and responsibility as Adokam Home Care continues to grow.
