About the group

Gratitude In Home Care is a registered aged care provider delivering Support at Home services built on transparent pricing, choice of worker and continuity of care. NorthPoint Disability is a registered NDIS provider delivering disability supports, including high-intensity supports, along with DVA community nursing. Both are part of one Preston-based group. We measure success by clients who stay, families who trust us and staff who want to keep working with us.

Purpose of the role

The Clinical Care and Compliance Lead keeps every client clinically safe and every client file compliant, across both brands. The role leads clinical assessment, care planning input and review, clinical risk and incident management, and clinical governance; owns client compliance against the Aged Care Act 2024, the Strengthened Aged Care Quality Standards and the NDIS Practice Standards; and builds the clinical capability of the workforce through training, competency assessment and education on matching clients to the right support.

The role sets the standard and audits against it. It does not roster, allocate shifts or choose individual workers; it defines the clinical and matching criteria the Service Coordinator applies, and teaches the team to apply them well.

Key responsibilities1. Clinical assessment, care planning establishment and review

  • Lead the clinical assessment and review cycle for Support at Home, NDIS and DVA community nursing clients, including complex care, wound, continence, pain, falls, nutrition and hydration, cognition and dementia, behaviour, medication and palliative and end-of-life needs.
  • Provide the clinical input to each care plan developed by the Client Care Partner so that clinical need, client goals, approved classification or NDIS plan and budget align; sign off the clinical elements of the plan.
  • Provide direct nursing care and clinical education to clients, families and carers where required, including DVA Community Nursing clinical assessment and care plans within each episode of care.
  • Monitor and evaluate clinical outcomes and deterioration; escalate and adjust care promptly; manage clinical risk for at-risk clients with documented mitigation and follow-up.
  • Liaise with GPs, allied health, hospitals, dementia, continence, palliative and specialist services for integrated care; support hospital discharge, transition between providers and the Restorative Care and End-of-Life pathways.
  • Support advance care planning, dignity of risk and supported decision-making conversations, and document them.

2. Funding knowledge and budget utilisation

  • Apply working knowledge of Support at Home (classification levels, quarterly budgets, care management funding, Restorative Care and End-of-Life pathways, Assistive Technology and Home Modifications scheme, co-contribution rules), NDIS plan types, support categories and Pricing Arrangements, and the DVA Community Nursing episode-of-care model.
  • Establish recommendations based on unmet needs for client budget utilisation, and work with Client Care Partners and Service Coordinators to address under- and over-utilisation, unspent funds and service mix so every client receives the full benefit of their entitlement.
  • Identify when a client's needs have changed and initiate reassessment, classification review or Support Plan review through My Aged Care, or a plan reassessment request with the NDIS.
  • Provide the clinical justification for assistive technology, home modifications, nursing and allied health within the client's funding, and for NDIS high-intensity supports.
  • Ensure services delivered are clinically justified, within the approved plan and documented to a claimable standard; return anything that is not to its owner before Finance claims it.
  • Contribute clinical input to funding, pricing and compliance discussions with management.

3. Client compliance

  • Own compliance of every client file: service agreement signed on or before the first service, informed consent, intake and clinical assessment, care plan, risk assessments, and documentation that meets program and claiming requirements.
  • Run the pre-start compliance check on every new client file submitted by the Client Acquisition Officer and return gaps for correction before the first service.
  • Maintain the client-level evidence required by the Strengthened Aged Care Quality Standards and the NDIS Practice Standards (including the high-intensity daily personal activities, medication management and, where applicable, behaviour support and restrictive practice modules).
  • Oversee NDIS restrictive practice obligations where they arise: behaviour support plan in place, authorisation, monthly reporting and elimination planning with the behaviour support practitioner.
  • Monitor and support Support at Home start-window and service-agreement compliance alongside the Client Acquisition Officer.

4. Incident management and safeguarding

  • Lead the incident management system for both brands: recording, assessment, immediate response, investigation, open disclosure, corrective actions and trend analysis.
  • Meet reportable incident obligations: Serious Incident Response Scheme (Priority 1 within 24 hours, Priority 2 within 30 days) and NDIS reportable incidents (24 hours or 5 business days as applicable), with the Operations Manager.
  • Lead medication safety, falls prevention, pressure injury prevention, infection prevention and control (including outbreak management) and manual handling programs.
  • Safeguard clients from abuse and neglect; apply the Aged Care and NDIS Codes of Conduct and worker screening requirements; refer concerns through the correct channels.
  • Provide the clinical element of complaint responses and service recovery, working with the Service Coordinator and Client Care Partner.

5. Clinical governance, quality and audit readiness

  • Lead clinical governance for the group: clinical policies and procedures, clinical audit calendar, quality indicator collection and reporting (QI Program, Star Ratings), and continuous improvement plan.
  • Own audit readiness for the Aged Care Quality and Safety Commission and NDIS approved quality auditors: evidence map by Standard, gap assignment to process owners, self-assessment and corrective actions.
  • Monitor changes to the Aged Care Act 2024, the Strengthened Quality Standards, Support at Home program rules, NDIS Practice Standards and DVA requirements that affect care and compliance; update policies and brief staff; coordinate with the Digital Marketing Specialist on public messaging.
  • Chair the weekly clinical and care review and the quarterly compliance and audit readiness review; prepare the monthly clinical, compliance and utilisation report for the Operations Manager.
  • Contribute to the improvement of clinical systems, documentation tools and workflows.

6. Workforce training, upskilling and matching education

  • Develop, deliver and present training to upskill support workers and Enrolled Nurses: wound care, medication support, manual handling, falls prevention, dementia and behaviour support, infection control, clinical observation, recognising deterioration, high-intensity supports and documentation.
  • Design and run induction, buddy shifts and clinical competency assessments for new and existing staff; maintain training and competency records; ensure mandatory training is current.
  • Identify skill gaps from incidents, audits, client feedback and supervision and translate them into toolbox talks, workshops and refresher sessions.
  • Set the clinical and matching criteria for pairing clients with workers (clinical need, risk, skills and competencies required, cultural and language fit, continuity) and run the monthly matching education session for the Service Coordinator, Client Care Partner and Client Acquisition Officer. The Service Coordinator applies the criteria; this role audits the outcome.
  • Supervise, mentor and clinically support Enrolled Nurses and support workers, and support a respectful team culture that keeps good workers with us.

7. Client and staff retention

  • Act as the trusted clinical point of contact for clients and families; respond to clinical enquiries, concerns and internal referrals within one business day.
  • Lead the clinical onboarding of new clients with the Client Acquisition Officer and Client Care Partner so the first weeks set the tone for a long relationship.
  • Work with the Client Care Partner on early-warning signs of clinical dissatisfaction or disengagement and resolve them before they become complaints or exits.

Selection criteriaEssential

  • Current registration as a Registered Nurse (Division 1) with AHPRA / the Nursing and Midwifery Board of Australia.
  • Substantial experience in aged care, community or in-home care, including clinical leadership and care coordination.
  • Working knowledge of the Support at Home program (or Home Care Packages with willingness to transition), aged care funding, care management and budget utilisation.
  • Strong understanding of the Strengthened Aged Care Quality Standards, the Aged Care Act 2024, clinical governance, SIRS and NDIS reportable incident requirements, and accreditation.
  • Exceptional clinical assessment, decision-making and risk management skills.
  • Demonstrated ability to lead, mentor and motivate a multidisciplinary team, and experience developing and presenting training and delivering competency assessments to support workers and Enrolled Nurses.
  • Demonstrated knowledge and application of continuous quality improvement and audit processes.
  • Excellent written and verbal communication, with the ability to build rapport across cultures and with families, GPs and external providers.
  • Proficiency in clinical documentation, client management systems and Microsoft 365.
  • Knowledge of Equal Opportunity, Disability Services and Occupational Health and Safety obligations as they apply to service delivery.
