Job Summary:

The Market Development Manager serves as a primary liaison between prospective clients, referral partners, and internal care teams. This position ensures prospective families receive timely, compassionate guidance and that service delivery is aligned with Home Matters’ standards of excellence. The CSM also drives business development efforts and cultivates strong, sustainable referral networks across the North Denver region.

Required Qualifications

  • Passion for senior care and supporting vulnerable populations
  • Excellent communication and interpersonal skills
  • Previous experience in home care, home health, hospice, or related fields
  • Established professional network across senior living, healthcare, or community-based organizations
  • Proven success generating new business and converting referrals
  • Strong sense of personal accountability and follow-through
  • Competency with CRM tools and willingness to learn new systems
  • Valid driver’s license and reliable transportation
  • Ability to meet all background and compliance requirements
  • Knowledge of Colorado home care regulations (or rapid ability to learn)
  • Availability to participate in the monthly on-call rotation

Essential Duties

  • Respond promptly to A Place for Mom (APFM) leads and referral inquiries
  • Conduct follow-up communications with prospects and partners
  • Collaborate with the Executive Director on high-priority referral accounts
  • Promote and support implementation of Sensi AI
  • Maintain accurate documentation of outreach efforts in HubSpot
  • Complete client intakes, assessments, and care planning tasks
  • Enter care plans and scheduled tasks into WellSky
  • Maintain regular communication with clients and families
  • Serve as a professional representative of Home Matters in all interactions
  • Provide backup client care to ensure continuity of services when needed
  • Perform additional duties as assigned

Operational Expectations

  • Standard schedule Monday–Friday; daily huddle at 8 AM
  • Timely return of completed client documentation
  • Ongoing collaboration with internal teams
  • Consistent engagement with referral sources
  • Full compliance with documentation and reporting requirements

Compensation: Base salary of $75,000 + commission
