About the Role The Engagement Specialist is a key relationship-builder and community representative in Matter Health’s care model. In this role, you will engage older adults in assigned communities, support patients in navigating their healthcare journey, and drive efforts in connection, outreach, enrollment, and retention. This highly interactive, field-based position is perfect for someone who enjoys building trust, connecting face-to-face, and making a meaningful impact within underserved communities. Engagement Specialists work closely with both operational and clinical teams to ensure patients are engaged, supported, and connected to care. The ideal candidate is compassionate, outgoing, organized, and thrives in a fast-paced environment where relationship-building and community engagement are central. What You’ll Do Build trusted relationships with older adult patients and community members Maintain a visible and consistent presence within assigned communities Support patients through their care journey, including outreach, scheduling, enrollment, and follow-up Educate patients on Matter Health’s care model and guide them through available services Conduct outreach through in-person visits, phone calls, and community events Partner closely with clinical and operational teams to support ongoing patient engagement and retention Identify barriers to care and support patients with empathy, compassion, and urgency Track outreach activities and patient interactions using Salesforce and internal systems Serve as a trusted community resource and advocate for patients Build awareness and foster engagement within local communities and housing sites What We’re Looking For 1–2 years of experience in community outreach, healthcare, customer service, hospitality, sales, or other relationship-focused roles Strong communication, relationship-building, and interpersonal skills Compassion for seniors and underserved populations Organized, proactive, and comfortable working in a fast-moving environment Ability to work independently while collaborating closely with cross-functional teams Comfortable working in community-based and field environments Valid driver’s license and reliable transportation required Experience with Salesforce, healthcare outreach, Medicare populations, or community engagement is a plus Bilingual Spanish is a plus Why Matter Health?
