Job Description:

QUALIFICATIONS:

  • B.A. in Behavioral Sciences or related field preferred, with experience working with adults with intellectual and developmental disabilities preferred.
  • Alternative education and experience may be substituted for degree requirements.
  • High School Diploma and valid driver’s license required.

BASIC PERFORMANCE EXPECTATIONS:

  • Being punctual and dependable.
  • Communicating with supervisor regarding relevant concerns, changes, and needs.
  • Remaining alert and focused while providing services.
  • Requesting assistance and guidance when needed.
  • Working cooperatively with individuals served, families, coworkers, and other providers.
  • Completing assigned responsibilities and required documentation.
  • Maintaining professional boundaries and conduct.
  • Attending required training, supervision, and meetings.
  • Following agency policies, procedures, and applicable regulations.

JOB DUTIES:

  • Assist in the development, implementation, monitoring, and ongoing review of individualized service plans/life plans and related goals.
  • Provide individualized support in accordance with each individual’s service plan/life plan, authorized services, needs, preferences, and goals.
  • Assist individuals in developing and maintaining skills that promote independence, self-direction, choice, and successful community living.
  • Assist individuals to access, manage, coordinate, and maintain community resources and supports, including medical, behavioral health, employment, education, housing, benefits, transportation, and other services as applicable.
  • Facilitate opportunities for individuals to develop and maintain meaningful relationships, natural supports, community connections, activities, and valued community roles.
  • Assist individuals with daily routines and responsibilities, including scheduling, appointments, household responsibilities, organization, and other areas identified in their service plan.
  • Support individuals in maintaining safe and stable living environments and developing the skills necessary to manage their homes and personal responsibilities.
  • Assist individuals with transportation and community access, including transportation to appointments, services, employment, activities, and other community settings as appropriate.
  • Support individuals during medical, behavioral health, and other appointments as needed and assist with communication, follow-up, and coordination of recommendations.
  • Monitor and communicate concerns related to the health, safety, and well-being of individuals and follow established protocols when concerns arise.
  • Implement or support established health, medical, and medication-related protocols as applicable to the individual’s service plan, training, and agency requirements.
  • Assist individuals with coordinating and overseeing services and supports, including PCA services when applicable.
  • Participate in team meetings, service planning meetings, supervision, appointments, and other meetings related to individuals served.
  • Maintain timely, accurate, and complete documentation, service records, billing information, mileage, and other required records.
  • Communicate relevant information and changes in individual needs, circumstances, or service delivery to the Program Coordinator and other authorized members of the support team.
  • Maintain appropriate communication and coordination with individuals, families, DDS, MassAbility, providers, and other members of the individual’s support network.
  • Respond appropriately to emergencies and crisis situations and participate in the regularly scheduled on-call/beeper rotation as assigned.
  • Acquire and maintain all required certifications, training, and competencies.
  • Protect the rights, dignity, privacy, safety, and self-determination of individuals served and report suspected abuse, neglect, exploitation, misconduct, or other concerns in accordance with agency policy and applicable regulations.
  • Perform other related tasks as assigned.

Pay Range:

$22.07 - $22.07
