Job Description:
Clinical Case Management is a more intensive service than traditional case management. Individuals referred to the program may require a higher degree of support, most often provided in their homes or in the community.
The Clinical Case Manager supports young adults and adults with Autism, intellectual and developmental disabilities, and mental health needs. The role teaches and reinforces skills in problem-solving, social connection, executive functioning, and other areas relevant to each person’s goals. Services promote stabilization in housing, engagement, community connection, relationships, mental health, and progress toward future goals.
The Clinical Case Manager brings curiosity, uses motivational interviewing and engagement techniques, remains open to new ideas, and collaborates with Open Sky colleagues, families, funders, providers, and other community partners.

The Clinical Case Manager delivers services through a person-centered, trauma-informed lens and demonstrates a steadfast commitment to diversity, equity, inclusion, and belonging.
Responsibilities
- Establish mutually respectful partnerships with people served, families, staff teams, and other stakeholders.
- Provide mobile support to individuals with Autism who are referred by DDS Area Offices.
- Provide in-home, behavioral, clinical, and community-based support to individuals.
- Assist people served to access transportation and provide transportation as needed.
- Operate within the philosophy of Positive Behavioral Supports and assist with PBS training for staff teams, families, and other stakeholders.
- Train program participants and collateral supports, including staff, families, and supporters, in identified supports, interventions, and strategies.
- Actively participate and consult with internal and external teams regarding the unique clinical needs of program participants.
- Prepare individualized treatment plans with assigned individuals and families, as clinically indicated.
- Participate in treatment planning to develop effective and achievable goals and objectives.
- Complete assessments, treatment plans, and other required documentation within established timelines.
- Attend and participate in meetings, trainings, and supervision as assigned.
- Participate with the treatment team in completing comprehensive assessments and Individual Support Plans.
- Apply motivational interviewing and engagement techniques to build relationships and define goals.
- Facilitate groups, organize events, and support other Autism Services activities based on participant needs.
- Act as a liaison with treatment team members, including families, DDS, day supports, professional providers, and allied health providers, including the VNA.
- Maintain current and accurate files and databases for individuals served.
- Participate in crisis support as needed.
- Perform other duties as assigned.
Qualifications

Bachelor’s degree required.

Experience supporting people with Autism, intellectual and developmental disabilities, and mental health needs required.

Experience teaching skills related to problem-solving, social connection, executive functioning, and other areas relevant to young adults and adults with Autism preferred.

Strong written communication, collaboration, engagement, and organizational skills required.

Ability to provide mobile, in-home, and community-based services and transportation support as needed.
Pay Range:
$31.52 - $31.52
