About the role
For this ACF we are seeking an enthusiastic candidate interested in the analysis of routinely collected primary and secondary care data. They will be training (or applying to train) in general practice and will undertake academic training alongside this, particularly in their last 2 years of training. They will receive supervision from academics in both General Practice and Data Science. ACFs in GP have a four year training programme meaning that they spend 25% of their time in ST3 & 4 undertaking research. This post has entry points at ST1,2 and 3.
This program will utilise linked NHS hospital and primary care data to investigate the clinical consequences of medication changes initiated during hospital admissions. The successful ACF will develop a program of work exploring the critical transition point of hospital discharge to community care where there are medication changes initiated during a hospital admission (including new prescriptions, dose adjustments, and discontinuations). These medication changes have significant implications for post-discharge outcomes, which then need to be managed in primary care. A cohort of patients vulnerable to such harms will be chosen such as those with a) polypharmacy ( 5 medications prescribed), b) Multiple Long-Term Conditions ( 2 long term conditions), or c) trans people (chosen due to evidence that they may be less likely to interact with standard primary care).
